ABSTRACT

BACKGROUND: Severity of irritant diaper dermatitis (IDD) from diarrhea varies from patient to patient depending on the nature of feces and the number of bowel movements. The purpose of the present study was to compare the effectiveness of dexpanthenol and zinc oxide ointment with ointment base in the treatment of irritant diaper dermatitis from acute diarrhea in children by measuring transepidermal water loss (TEWL). MATERIAL AND METHOD: Forty-six children with diarrhea were prospectively, block randomized, investigator-blinded to receive dexpanthenol and zinc oxide ointment on one side and ointment base on the other side. TEWL was measured before and on days 1, 3, and 7 of treatment together with the assessment of severity score. The efficacy of treatment was defined by complete clearance of the lesion. RESULTS: TEWL in the treated and control side was not different before the application of the topical medication. In the present study, the efficacy of 5% dexpanthenol and zinc oxide ointment on D3 was 39% (18from 46 patients) compared to 32% in the ointment base side. On D7, the efficacy of the treated side was 58.7% and the ointment base side was 56%. The patients who still had skin lesions were those who had prolonged diarrhea. On the treated side, the mean of TEWL was lower than the control side on DI (p = 0.18) and had significant improvement on D3 (p = 0. 002). At the end of the present study, TEWL on the treated side was less than TEWL of the control side but it did not have statistical significance (p = 0.07). There was no rash or sign of abnormality on the treated side at the end of D7. CONCLUSION: In the treatment of lDD from acute diarrhea, 5% dexpanthenol and zinc oxide ointment significantly decreased TEWL in the treated side more than the ointment base on day 3 but the severity score was not significantly different on days 1, 3 and 7.

ABSTRACT

Prolonged exogenous administration of ACTH or hydrocortisone or its analogs results in a clinical pattern identical to the spontaneous disorder and is frequently referred to as Cushingoid syndrome. This syndrome is generally reported as a result of oral or parenteral steroids administration. Our report presents a five-month old baby with typical Cushingoid changes following local steroid application due to diaper rash. Clobetasol was used for diaper rash, but had not used systemic corticosteroid. Serum Cortical and ACTH was very low and no detectable. After discontinuation of local steroid, Skin complication recovered gradually and after 5 months, completely. Local corticosteroids may be high absorption and cause systemic complication such as Adrenal insufficiency and cushingoid syndrome

ABSTRACT

Introducción. La dermatitis de la zona del pañal (DZP), común en lactantes, se agrava frecuentemente con infecciones, principalmente por Candida albicans. Objetivo. Comparar la eficacia y tolerancia a la terbinafina en crema contra la de ketoconazol en crema en candidiosis de la zona del pañal. Material y métodos. Se realizó un ensayo clínico abierto, comparativo, aletorizado, separando 2 grupos, indicandoseles aplicar terbinafina en crema una vez al día durante 14 días al primer grupo y ketoconazol 2 veces al día, por mismo periodo, al segundo grupo. El seguimiento se realizó por 4 semanas. Resultados. Ingresaron 52 pacientes; 28 en el grupo de terbinafina, 24 en el ketoconazol; 28 del sexo masculino y 24 del femenino. Hubo un sólo caso de reacción adversa en el grupo de terbinafina y consistió en incremento del eritema y prurito. El resto de parámetros evolucionó en forma similar. Conclusiones. Los análisis estadísticos no reportan diferencia significativa. Se considera a la terbinafina como alterativa en una circunstancia clínica no estudiada antes

ABSTRACT

Foi estudada uma associaçäo de alúmen amoniacal/ácido salicílico/ácido bórico, em 30 crianças de idades entre 15 dias e 16 meses, portadoras de dermatite de fraldas, de graus leve ou moderado. O tratamento foi por banhos da regiäo perineal, coxas e abdome, durante 15 minutos, duas vezes ao dia, por sete dias. Avaliou-se comparativamente, na admissäo e após os sete dias de tratamento, o grau (ausente, leve, moderado, intenso) de cada manifestaçäo cutânea característica do quadro de dermatite de fraldas: eritema, edema, vesículas, bolhas. Dos 30 pacientes, 29 iniciaram o estudo com dermatite de grau moderado e 1 com grau leve. Aos sete dias de tratamento, 12 pacientes apresentavam pele normal, 10 dermatite de grau leve, 7 moderada e 1 de moderada a intensa. Em comparaçäo com o estado na admissäo, o tratamento produziu 90% de melhoras ou curas. Näo foram relatadas reaçöes adversas ao tratamento. Os banhos com essa soluçäo parecem ser uma boa opçäo para o tratamento da dermatite de fraldas, principalmente na sua fase aguda

ABSTRACT

Se evaluó la eficacia y tolerancia de tolciclato crema en 42 pacientes con candidiasis de la zona del pañal. Los resultados fueron excelentes en el 100% de los pacientes, en los cuales se obtuvo curación clínica y micológica en un periodo de una a dos semanas de tratamiento. El medicamento eliminó rapidamente la sintomatología y la tolerancia fue buena. Tolciclaro demostró una gran eficacia contra Candida albicans, y la aceptación del tratamiento fue muy buena
